Our doctors are passionate about healthy ageing and believe that age should not be a barrier to good health. We provide continuity of care to our patients going through complex health needs as they grow older and coordinate with their other health care providers and families (when necessary). We organize over 75yr Health Assessments and Home Medicine Reviews to get a better understanding of how patients are managing at home, identify any unmet needs, reduce the risk of medication error and improve medication literacy.
