Home Medicine Reviews

This service is provided by Andrew Clayton.

At our practice, we offer home medicine reviews for patients who require assistance managing their medications. A home medicine review is a comprehensive review of a patient’s medications by a pharmacist in the comfort of their own home.

During the review, our healthcare provider will work with the patient to identify any issues with their medications, including incorrect dosage or potential side effects. The provider will also make recommendations to improve medication management and ensure that the patient is receiving the best possible care.

A home medicine review can be particularly beneficial for elderly patients or patients with complex medication regimens. The review can help to identify and address potential medication-related problems and improve overall health outcomes.

To request a home medicine review, simply call our practice and speak to one of our staff members. We will arrange a suitable time for our healthcare provider to visit you at home and conduct the review. Our staff will provide you with all the information you need to receive a home medicine review and answer any questions you may have.

GP Chronic Condition Management Plans

We offer GP Chronic Condition Management Plans for patients with chronic or complex medical conditions.

In order for patients to access a GP Chronic Condition Management Plan they must be registered with MyMedicare and select their preferred GP who will be doing the plan  MyMedicare registration form

A GP Chronic Condition Management Plan is a written plan of action developed by your GP that outlines the management of your health condition and the treatments and services you will receive to help you manage your health.

The plan is tailored to your individual needs and may include a range of healthcare services, such as referrals to specialists, allied health professionals, or community support services. The plan also outlines the goals and targets for managing your condition and the steps you can take to achieve these goals.

These are an effective way to ensure that you receive the necessary care and support to manage your chronic condition and improve your overall health outcomes. The plan is reviewed regularly by your GP to ensure that it remains up-to-date and relevant to your current health needs.

Your GP will work with you to develop a plan that meets your individual needs and supports you in achieving optimal health outcomes.
