Palliative Care
Palliative care is person and family-centred care provided for a person with a life limiting illness, for whom the primary goal is to optimise the quality of life.
GP’s working in the clinic are ideally placed to address the complex issues faced by patients and their families as we have broad knowledge, long standing relationship with patients and their families, ability to carry out home visits and communicate and coordinate with other health care providers (e.g. Community Palliative Care Nurses and Palliative Care Consultants)
